Fees:   

(taxes are included for all fees)

Athletic Therapy     $60.00


Custom Orthotics     $300.00

            (Includes; assessment, casting, orthotics and fitting)



Gait Analysis and Technical Instruction for Runners      $60.00

           

Includes two 30 minute individual sessions.


These sessions are offered after an Athletic Therapy assessment, treatment, and consultation has been completed.


           


WCB and MPIC claims are accepted. *Patients must pay $10.00 per visit. The remainder of the fees will be billed directly to WCB or MPIC.



Payments:

  1. Cash or cheque only, due at each session.



Note:

  1. Athletic Therapy is covered by most insurance companies, however, individual policies may vary. Contact your insurance provider and ask specifically if Athletic Therapy is covered on your plan.
